Long-term home oxygen: a UK perspective.

چکیده
1 There is little doubt that children with chronic hypoxemia benefit from supplemental oxygen, which, when appropriate, can be administered outside the hospital setting. Long-term home oxygen is now a well recognized form of therapy; however, as is often the case in pediatric practice, evidence for its benefit is not always convincing. This was highlighted in the 2009 British Thoracic Society (BTS) guidelines for home oxygen in children,1 in which only 15% of the recommendations were graded A, B or C.2 This fact has inevitably led to variable practice, lack of consensus, and possible misuse of supplemental oxygen. In this issue of Jornal de Pediatria, Munhoz et al.3 present a retrospective cohort study of 165 patients from a single pediatric center in São Paulo, Brazil, receiving home oxygen during the 8-year period of 2002 to 2009. Their paper reviews the underlying diagnoses of patients on home oxygen, compares those with and without secondary pulmonary hypertension, and performs a cost analysis. This editorial will give a UK perspective on these issues.

Delivery systems for long-term oxygen therapy.
Table 5 summarizes my current perspective on home oxygen delivery systems in the United States. As I already indicated, scoring for cost and labor may be very different in other countries. As noted, all things considered, today the most reasonable system for home oxygen therapy is the concentrator. Problems with FIO2, liter flow, and portability are clearly overshadowed by cost, labor, ease of ...

1. One of the areas that remains uncertain is the maximum inspiratory oxygen fraction (FI,O2) that should be made available to patients on long-term oxygen therapy (LTOT) at home who require high levels of oxygen, e.g. end-stage interstitial lung disease (ILD) patients. In the situation of increasing breathlessness, but with a limited evidence base, what is your recommendation? Although ambulat...

UNLABELLED "Non-delivery" home oxygen technologies that allow self-filling of ambulatory oxygen cylinders are emerging. They can offer a relatively unlimited supply of ambulatory oxygen in suitably assessed people who require long-term oxygen therapy (LTOT), providing they can use these systems safely and effectively. This allows users to be self-sufficient and facilitates longer periods of tim...
